Submit the following materials.
- Application for Oklahoma School Standard Teaching Certificate (Form)
- Official transcripts that denote degree earned and date conferred
- Copy of teaching certificates along with Proof of Teaching Form
- Processing fee
- Fingerprint background check fee (Fingerprint Information)
